Pre-filing Bankruptcy Counseling
Internet Based Course - Choices

If you are interested in completing your pre-filing bankruptcy counseling on the internet, our internet based course “Choices” may be what you are looking for.

Please follow the instructions carefully and return these forms to us:

1. Affidavit for Pre-filing Bankruptcy Counseling
2. Privacy Notice
3. Statement of Counseling Services
4. Copy of drivers license or state issued ID card
5. $50 registration fee.

Internet Portion

• The education portion of the financial management course is completed on the Internet.
• A budget is completed online.
• A financial net worth statement is completed online.
• Pro’s and Con’s of Chapter 7 and Chapter 13 are discussed.
• Alternatives to Bankruptcy are reviewed.

After completing this portion, you submit your budget and financial net worth statement to CCCS.

Telephone Follow-Up Counseling Session

A certified Credit Counselor will contact you by phone or email to set up a telephone counseling session to review the following:

• Budget analysis & financial net worth.
• Review of the current financial situation.
• A discussion of the factors that caused the financial condition and assistance in developing an action plan to address the problems.

This session will be done within 3 business days of you completing your internet education portion.

Upon completion of the telephone counseling session, the certificate of completion will be sent to you by mail and email.

Fees
The fee for the pre-filing bankruptcy counseling session is $50 for either an individual or joint filing. This fee can be waived or reduced in limited hardship circumstances. Please call 1-800-355-2227 for more information regarding this.
